Optus network failure impacted triple-0 calls in several states where patients died after 'totally unacceptable' lapse

Three people have died, including an eight-week-old baby, after a technical failure on the Optus network meant triple-0 calls were unable to go through.

Optus CEO Stephen Rue said "approximately 600 customers were potentially impacted" during a network upgrade.

"We are conducting a thorough investigation and once concluded, we will share the facts of the incident publicly," Mr Rue said.
